If your plan uses the AIG National Dental NetworkSM, the CAREINGTON PPO dental network or the EyeMed Vision network and your provider is not a member of that network, you can nominate your provider for membership.
Simply click the appropriate nomination form, print and complete it, and then submit it per instructions indicated on the form. Your provider will be invited to participate in the network.
