Freedom to select any dentist with a full range of dental services.
- Association members, spouses and dependent children to age 19 (25 if full-time student) are eligible
- Insureds must satisfy a $50 deductible ($150 family maximum), after which the balance of the covered charges will be paid up to a maximum listed in the Schedule of Benefits
- Includes coverage for preventative and basic services
- Six-month waiting period for endodontics and oral surgery
- Twelve-month waiting period for periodontics, major restorative, removable prosthetics and fixed bridge services
